We would like to welcome you to our practice. We
are a group of internal medicine physicians, we are considered primary
care physicians, and we specialize in adult medicine seeing patients
ages 18 and above. We differ from family physicians because we treat
only patients over 16 years of age. Your healthcare is a priority and if
you have any concerns please feel free to let us know so we can help in
any way possible.

you visit our office

| When you come to our office you will be
asked to bring a photo ID and your insurance card along with any co pay
that you are responsible for. If you are able to print off the new
patient forms shown on this web site, we ask that you fill these out
completely and bring them with you, or come in 10 minutes before your
appointment time to fill them out while in the office. Also, please
bring all medications that you are currently taking so we can go over
them with you. |

Prescription Refills

We request patients be seen annually for routine medication refills.
We typically do NOT call in antibiotics over the
phone. You should be seen to be treated.
We do NOT prescribe or refill controlled
substances over the telephone. This includes such as narcotic pain
medication.
Please have the following information available
when calling the prescription line at 614-508-2672:
- Your name and date of birth,
- Name and strength of medication to be
refilled,
- How often you take the medication and quantity
you usually receive,
- Pharmacy phone number.
