Patients are seen by appointment only. Please call (937) 578-2796 or (866) 547-2136 to schedule an appointment. Emergent visits are scheduled as soon as possible. Non-emergent visits are scheduled for the next available opening. 24 hours notice when canceling an appointment is appreciated. Your insurance policy usually required an office visit with your
primary care physician in order to obtain a referral to a specialty
physician. Primary care physician referrals may extend to
out-patient testing such as laboratory and radiology. |

| We accept most major medical plans. We
attempt to minimize your medical costs and specialize in preventive
medical care. By reducing billing expenses, we can help lower your health costs. Therefore, payment is requested at the time of your visit. This means you are responsible for insurance co-payments and any balances that your insurance does not pay, at the time of service. Please ask our staff if you have any questions or need assistance. The centralized billing department of Central Ohio Primary Care can assist you with any questions or concerns regarding your account. Their phone number is (888) 877-2672.Customer Service hours are 8:30 AM to 5:30 PM Monday through Thursday and 8:30 to 4:00 PM on Friday. |
