Our mission is to serve with competence the acute and chronic health care needs of our community. We believe in a personal and family approach to health care. We look forward to helping you be well. It does take a team to build and maintain good health.

Office Hours

Our office hours are Monday through Friday, 8 AM to 5 PM. We have "Sick Bay" walk-in hours from 8 AM to 9 AM for our patients.

Medical attention is available 24 hours a day, everyday of the year, including holidays. If you have an emergency or serious medical condition, please call the Memorial Hospital operator at (937) 644-6115 or (800) 686-4677 and they will contact the on-call provider. The on-call provider will return your call as promptly as possible.

Appointments, Refills, Questions

You can speak with a nurse Monday through Friday, 8 AM to 5 PM regarding: prescription refills, scheduling appointments, requesting a referral, or discuss any medical concerns. If all of the nurses are on the phone with other patients, you will have the opportunity to leave a message with their voice mail. The nurses check their voice mail routinely and try to return your calls promptly. We thank you for your patience and understanding.

When leaving a message please clearly state:

Your full name (please spell your first and last name) and the patient full name
Date of birth
Home telephone number or a telephone number where we can return your call and appropriate time to call if needed.
Medication allergies
Pharmacy name and phone number

If you are calling in for a prescription refill, please instruct us if the prescription is to be called to a pharmacy or if must be written for mail-in services. We kindly ask that you give us 48 to 72 hours notice for prescription refills. Please check with your pharmacy the following business day after 5 PM.

We will not fill prescriptions for narcotics, many pain medications or controlled substances outside of regular office hours.

Referrals

It is important that you review your insurance company's policy on referrals and benefits for services that are not provided us. We refer patients to many specialists. Most insurance companies require the primary care provider (us) to make the first appointment with the specialist. It is extremely important to verify the specialist is a designated provider by your insurance company. If you feel you need to see a specialist, please contact our office so that we can schedule an appointment to discuss any referrals. In the event that you seek care at any emergency department or an urgent care center, please notify our office on the next business day as your insurance carrier may require additional documentation.

Insurance / Medicare / Medicaid

Our patients are encouraged to ask any questions regarding billing and insurance. For most services, we request payment at the time of service. However, if special arrangements need to be made for an extended payment plan, we are willing to work with you. We accept most major insurance plans including assignment from Medicare. In order to verify our providers are covered by your plan, please feel free to contact us. We ask for current insurance information and a copy of your insurance card at every visit to facilitate any referrals or other administrative health care needs. Thank you for your assistance.
