Appointments can be scheduled by calling (614) 457-4806

If you are referred to a specialist, it is your responsibility to verify they are participating providers on your insurance plan and if a referral is needed.  If a referral is needed, you may call 457-4806, option 4.  Please call in advance of your scheduled appointment with the specialist. We will be unable to do a referral on the same day as the appointment unless it is an emergency appointment.

  We are providers for most major health plans. Please bring your insurance card with you each time you visit our office. Copays are collected at the time of check-in.  If you do not have insurance, you will be expected to pay at the time of service, unless other arrangements have been made in advance.  If you have a question regarding your insurance coverage, please call the customer service number located on your  insurance card.
