Our office hours are Monday through Friday, 8 AM to 5 PM. We have "Sick Bay" walk-in hours from 8 AM to 9 AM for our patients.

Appointments & Referrals
You can speak with a nurse Monday through Friday, 8 AM to 5 PM regarding: prescription refills,
scheduling appointments, requesting a referral, or discuss any medical concerns. If all of the nurses are on the phone with other patients, you will have the opportunity to leave a message with their voice mail. The nurses check their voice mail routinely and try to return your calls promptly.

Billing & Insurance
Our patients are encouraged to ask any questions regarding billing and insurance. For most services, we request payment at the time of service. However, if special arrangements need to be made for an extended payment plan, we are willing to work with you. We accept most major insurance plans including assignment from Medicare. In order to verify our providers are covered by your plan, please feel free to contact us. We ask for current insurance information and a copy of your insurance card at every visit to facilitate any referrals or other administrative health care needs.

After Hours
Medical attention is available 24 hours a day, everyday of the year, including holidays. If you have an emergency or serious medical condition, please call the Memorial Hospital operator at (937) 644-6115 or (800) 686-4677 and they will contact the on-call provider. The on-call provider will return your call as promptly as possible.
