    The Central Ohio Primary Care COPD Management program is aimed at patients with Chronic Obstructive Pulmonary Disease (COPD). This initiative falls in line with Population Health Management, where programs are offered to keep patients healthy and out of the emergency room and hospital.

    COPC patients are now able to schedule an office visit with Becky Hoberty, COPC’s respiratory therapist.  Becky has over 30 years of experience as a director and coordinator of pulmonary rehabilitation programs in both in-patient and out-patient office settings.

    As a respiratory therapist, Becky offers care through patient assessments, education and preventative work. Those seeking an appointment typically have trouble with recurring infections and hardships with day-to-day management.

    “I don’t like the concept that COPD is a death sentence, because it’s not,” she said. “It’s chronic. It’s something that we do have to understand and manage. It’s not going to go away but it can be so well managed.”

    Hoberty assesses each situation by making sure patients are on the proper medications and inhalers. She also helps teach each patient how to use their prescribed medications and inhalers properly.  Infection recognition, prevention, control, and early intervention are all steps Hoberty takes to help keep patients out of the emergency room.

    Most of this can be performed at your primary care physician’s office or at a SameDay Center.  In addition to one-on-one meetings, COPC offers free COPD Management classes on a monthly basis.

    One-on-one assessments are between 75-90 minutes. It is not limited to one visit. Patients are able to follow up as they would with any specialist. Most patients are seen at least three times.  Free group classes are also available but are not as patient-specific as the one-on-one meetings.  Classes are more of a support and overview session.  A physician referral is not needed to attend the free group classes.
