About Amy R. Kelley, M.D.

Appointments

Appointments can be scheduled by calling (614) 459-2950. We ask that you call in advance to schedule your appointments. If you are unable to keep the appointment, please give 24 hours notice in order that another patient may be given that time. We reserve the right to charge for recurrent missed appointments. There are sick visits available each day for your convenience, however they do fill up quickly.  If you are needing a same day appointment, we recommend you call before 10:00.  A 24 hour cancellation policy is in effect. You will be asked to reschedule your visit if you are more than 10 minutes late in order for the doctors to provide efficient care and minimize waiting time for all patients.

In order for the primary care physician to have adequate time to address your concerns, it is crucial to inform the receptionist of the exact nature of your visit. Due to time constraints, the physicians may limit each visit to only two issues. There are several things you can do to help us stay on schedule. When calling to schedule an appointment, be specific about the nature of your visit. A “routine appointment” is not designed to exceed fifteen minutes. Finally, we ask you for your understanding and patience when we are running behind schedule. Emergencies occasionally disrupt the schedule. When this happens, we will try to get back on schedule as soon as possible.

When You Visit

Please present your insurance card at each and every appointment.

Please Note

If you have any complaints during your well visit (PE/PAP) this will be billed separately and you may incur additional charges on same day. Typically this will include your copay and any coinsurance.  We prefer to keep well visits and sick visits (any complaint) separate and you will be told by the staff you will have to make another appointment.  This is done to avoid billing issues and extended wait times for patients.  If the doctor finds that an issue has to be addressed today or agrees to see you for a sick issue, your insurance will be billed as such.

Prescription Policies and Information

Prescriptions and refills will be handled at scheduled office visits and enough medicine will be prescribed to last at least until the patient’s next appointment. Since we expect to fill prescriptions at the office visit, running out may be a signal that it is time to call our office to schedule an appointment. We request patients be seen annually for routine medication refills. If your prescription insurance plan requires a prior authorization for a medication, you will be responsible for obtaining a Prior Authorization Form from your insurance company for your physician to complete. While our office staff and physicians do their best to help patients obtain their maximum prescription benefits, insurance plans, including drug coverage plans, are a contract between the patient and the patient's insurance company.

If you need to change pharmacies or change your prescription to mail order, the law in the State of Ohio permits non-expired prescriptions to be transferred to the new pharmacy without charge or interruption. In the case of most mail order pharmacies, they disregard this provision and require prescriptions to be re-written. In the event that this is preferred by the patient BEFORE their next routine visit, the patient has a couple of choices:

  1. Make an appointment to see the doctor for repeat refills
  2. Wait for their next regular visit to change pharmacies
  3. Pay a fee for the patient's physician to review their medical records and re-write the prescription - which will then need to be picked up at the office. Lost prescriptions for controlled medications (such as pain medications, sleeping aides, etc.) will not be replaced.

We discourage the practice of calling in prescriptions as it does not allow the patient or the doctor to re-evaluate the appropriateness of the prescription, and follow the condition for which the medication has been prescribed. We do not accept calls from pharmacies to refill medications. When pharmacies call us, we request contact directly with the patient.
