Office Hours
Our office is open from 8:00 AM to 5:00 PM on Monday, Wednesday, Thursday, and Friday. We are open from 9:00 AM to 5:00 PM on Tuesday. If you have an urgent medical need after hours, please call the Medical Bureau at (614) 784-4663, or the office at (614) 527-2562 to receive further instructions for reaching the physician on call. If you have a life threatening emergency, call 911.

We are located at 3535 Fishinger Blvd in the Mill Run area. We are located on the second floor just to your left after you get off the elevator. From I-270 take the Hilliard/Upper Arlington exit and turn East toward Upper Arlington on Fishinger Road. Follow Fishinger Road past the fire station and turn right. Our building is on your left after you turn off Fishinger. From Upper Arlington take Fishinger West across the bridge. Just after the second stoplight at Smiley road turn left. Our building is on your left just after you turn off Fishinger.

Appointments & Referrals
Appointments can be scheduled by calling (614) 527-2562. We ask that you call in advance to schedule your appointments. If you are unable to keep the appointment, please give 24 hours notice in order that another patient may be given that time. We reserve the right to charge for recurrent missed appointments.

Billing & Insurance
Our doctors desire that you take an active role in your health care, which includes all matters concerning your medical bill. We ask that you read and sign our billing information sheet at your initial visit to our office. To maintain your account in good standing, all charges are due at the time of service. Extended payment plans need to be discussed in advance. Please let our business staff know if you have any questions concerning your billing payments.
Our practice accepts many different insurance plans. Please call our business office with questions about specific insurance coverage. We will be happy to submit your insurance claim form to those insurance companies with whom we have contracts. To keep billing costs down, we ask that you pay any applicable co-pays at the time of service. We strongly encourage our patients to review their insurance policies in depth to fully understand what benefits are covered.

Due to the increasing amount of time required to fulfill your refill requests we will no longer take prescription refill requests over the phone or by fax. This change will allow our nursing staff to focus on patents being seen in the office and attend to their medical needs and questions. It will also enable our reception staff to answer phones and schedule your appointments and referrals in a timely manner. Please use one of the following options to obtain your refills:
* Please obtain your prescription refills at your regularly scheduled office visit.
* You may send us a written refill request with a self addressed, stamped envelope.
* You may drop off a list of needed prescriptions that can be picked up or mailed out in a self addressed stamped envelope 48 hours after drop off.
You requests must include:
* The name and dosage of each medication.
* How often you take it.
* How many pills you wish to have: 30 or 90 day supply.

It helps if you save your non-urgent questions and direct them to the physician during your office visit. Otherwise, please call during the hours that your physician is usually in the office so that our staff and your physician can best meet your needs.
