Prescriptions & Refills
General Policy:
We request patients be seen annually for routine medication refills.
Typically, we do not call in antibiotics over the phone. You should be seen to be treated.
We do NOT prescribe or refill controlled substances, such as narcotic pain medication, over the telephone. Please have the following information available when calling the prescription line -(614-508-2672):
Your name and date of birth;
Name and strength of medication to be refilled;
How often you take the medication and quantity you usually get;
Pharmacy phone number

When you come to our office you will be asked to bring a photo ID and your insurance card along with any copay that you are responsible for. If you ar able to print off the New Patient Forms shown on this web site, we ask that you fill these out completely and bring them with you or come in 10 minutes early for your appointment to fill them out while in the office. Also, please bring all medications that you are currently taking so we can go over them with you.
