Appointments & Referrals
Our office requires a scheduled appointment to see a physician. You may contact the office at 614.326.1502 during normal business hours to schedule an appointment. We require at minimum a 24 hour notice to cancel an appointment to avoid a late cancel or missed appointment charge. For your convenience, you may contact the office after hours as well and leave a detailed message by pressing option ‘1’ on your phone. This voicemail box is to cancel an appointment only and you will need to call the office during normal business hours to reschedule. In the event of in climate weather, please call the office prior to leaving for your appointment in the event the office is closed.

Prescriptions & Refills
Maintenance prescription refills require a yearly office visit or more frequent visits if indicated by your physician. If your prescription insurance plan requires a prior authorization for a medication, you will be responsible for obtaining a Prior Authorization Form from your insurance company for your physician to complete. While our office staff and physicians do their best to help patients obtain their maximum benefits, insurance plans including drug coverage plans, are a contract between the patient and the patient’s insurance company.
Antibiotics, pain medications and routine refills are NOT typically called in after hours.
When calling the office for a refill, please have the following information for the secretary:
Your name/Patient Name
Drug name and strength
Quantity and directions
Name and phone number of the pharmacy
Lost prescriptions for controlled medications (such pain medications, sleeping aides, etc.) will not be replaced. If a written maintenance prescription is lost, a replacement fee will be charged.

You can reach the On-Call Physician during non-business hours for urgent problems that cannot wait until the office is open.
