If you cannot keep your appointment, we ask you to call 24-48 hours in advance. There is a charge for non-cancelled appointments and insurance companies do not pay for this charge. Prescriptions & Refills
Our Doctors give enough medication until they need to evaluate your condition again, this visit may include blood work to maintain proper dosing of that medication & its effect on your body. The easiest way to maintain your medication supply is to keep scheduled office visits so the doctors can refill your medications on time.
Please allow 24-48 hours to process request for medications. Select Option 2 and be prepared to give your name and the name of your medication(s), with the spelling, the dosage, pharmacy name & phone number.

Billing & Insurance
Our office policy is to provideyou with the maximum uinpatient care. This can only be done with the patient and physicians office being a TEAM to help provide you wilh all the benefits you are allowed under your insurance plan. We ask that you present your current, valid insurance card at every visit and we also ask for a photo ID to air i identy theft. Please provide these when checking in at the front desk.
One way to assit this process is to complete the paperwork prior to your visit. Many of our patient forms can be downloaded from our other web site www.fpcw.net. Please print out these forms, complete and bring with you at the time of our visit.
We accept VISA, MASTER AND DEBIT CARDS.

Because of your insurer's payment policy, we may have to complete your "wellness care" and your "illness or medication care" in two visits. If you have a health concern you want to discuss with your doctor during your "well visit", the doctor may decide to treat the problem and ask you to schedule another appointment for your "well visit".
