Billing & Insurance
We are providers for most major health plans. Please bring your insurance card with you each time you visit our office. Copays are collected at the time of check-in. If you do not have insurance, you will be expected to pay at the time of service, unless other arrangements have been made in advance. If you have a question regarding your insurance coverage, please call the customer service number located on your insurance card.
