Our Philosophy for Diabetes Care

The good news about diabetes today is that it is completely controllable. As an individual with diabetes, you are responsible for 98 percent of your care. Learning how to manage diabetes will help you live a healthy life. Everyday decisions are made related to when to check blood sugar levels, what to eat, when to exercise and when to take medications.

We believe in the human approach to diabetes self-management. Every person’s diabetes is different and we can help you personalize your care plan. Our program will provide current education on the disease process, diabetes myths, medications, healthy eating, exercise recommendations and resources.

Available Programs

Basic Diabetes Self-Management Education
This monthly class series meets for four, two-hour sessions. It is designed to enable you to learn and use the skills necessary to self-manage your diabetes. We focus on basic principles related to self-monitoring blood glucose, meals, medications and exercise.

Think Like a Pancreas
This monthly class series meets for four, two-hour sessions. It is designed for individuals who use insulin to treat their diabetes. This class will teach the principles needed to adjust insulin based on blood sugar levels and food intake. It also will reinforce general guidelines for diabetes self-management.

Starting Insulin
The purpose of this one-time session is to teach you how to administer insulin as well as general information on insulin therapy.

To attend Central Ohio Primary Care's Diabetes Management classes, you will need a referral from your doctor. Download our Physician Referral Form here
