Diabetes Management

It Is Possible to Thrive With Diabetes

COPC Innovative Diabetes Management was formed to provide education and support to patients living with Adult Type 1 and Adult Type 2 diabetes. We’ll teach you the skills you need to manage your disease and reduce its potential long-term effects.

Every member of our team is also thriving with diabetes, so we understand the challenges you face every day. When you join one of our classes, you will be surrounded by men and women just like you who are looking for ways to enhance their health and well-being through education. Not only will you learn more about living with diabetes, but you’ll have fun and gain confidence as you take control and start feeling your very best.

How to participate in our Diabetes Self-Management Education Programs

We require a physician referral in order to work with your health insurance company. We recommend that you contact your insurer directly to determine your specific coverage. Our team acts as a liaison between you and your physician, so that he/she can track your progress.
