Prepare For Your Appointment
Appointments & Referrals
Our office requires a scheduled appointment to see a physician. You may contact the office at (614) 326-1502 during normal business hours to schedule an appointment. For your convenience, you may contact the office after hours as well and leave a detailed message by pressing option ‘1’ on your phone. This voicemail box is to cancel an appointment only and you will need to call the office during normal business hours to reschedule. In the event of in climate weather, please call the office prior to leaving for your appointment in the event the office is closed.
Prescription Policies and Information
Maintenance prescription refills require a yearly office visit or more frequent visits if indicated by your physician. If your prescription insurance plan requires a prior authorization for a medication, you will be responsible for obtaining a Prior Authorization Form from your insurance company for your physician to complete. While our office staff and physicians do their best to help patients obtain their maximum benefits, insurance plans, including drug coverage plans, are a contract between the patient and the patient’s insurance company. Antibiotics, pain medications and routine refills are NOT typically called in after hours. When calling the office for a refill, please have the following information for the secretary: Your name/Patient’s name, drug name and strength, quantity and directions, name and phone number of the pharmacy. Lost prescriptions for controlled medications (such as pain medications, sleeping aides, etc.) will not be replaced. If a written maintenance prescription is lost, a replacement fee will be charged.
Late Cancellation and No-Show Policy
We understand that plans can change. If you need to cancel or reschedule your appointment, we're here to help.
To keep appointments available for all patients and reduce wait times, please contact us at least 24 hours in advance if you cannot make your scheduled visit.
What Counts as a Late Cancellation or No-Show?
Late Cancellation: Canceling or rescheduling your appointment less than 24 hours before the scheduled time.
No-Show: Missing your appointment without notifying us at least 24 hours in advance.
Fees for Missed Appointments
Appointments not canceled at least 24 hours ahead of time will incur a $25 fee.
Please note: This fee is not covered by insurance and will be billed directly to you.
How to Cancel or Reschedule
You may cancel or reschedule your appointment in any of the following ways:
- MyChart: If you're enrolled, you can cancel or reschedule anytime through your account.
- Call the office: If we're unable to answer, please leave a voicemail with your full name, date of birth, appointment date/time, and the reason for cancellation.
Helpful Reminder
Please arrive 15 minutes before your scheduled appointment to allow time for check-in and any necessary paperwork. This helps us stay on schedule and ensures your visit runs smoothly.
