How to Get Started
Ask your COPC pediatrician about Next Step Young Adult Health at your next visit. They can answer your questions and schedule your first appointment before you leave the office. Next Step accepts the same insurance plans as other COPC primary care practices, so there’s nothing extra to sort out on that front.
Parents are welcome to stay involved early on. Next Step supports a gradual transition where young adults take on more ownership of their healthcare decisions over time. As with any adult patient, COPC follows applicable privacy laws regarding who can access medical information once a patient reaches adulthood.

No. You should continue seeing your pediatrician until it’s appropriate to transition to adult care.
No. You don’t need a referral or a new-patient registration when your pediatric practice transitions you. Your care moves to Next Step automatically.

Generally, yes, when clinically appropriate. Medication management is determined by your Next Step physician based on your medical history, current condition, and ongoing care needs. If you need a refill before your first appointment, we’ll take care of it and schedule the appropriate follow-up visit.
You should still establish care at Next Step Young Adult Health. Your physician will manage your primary care and coordinate with specialists as appropriate.
Part of the transition as you move from pediatric to adult medicine is beginning to take ownership of your own medical care. Next Step Young Adult Health supports this process by initially welcoming parent involvement, then encouraging you to exercise the freedom to make medical decisions on your own. Legally, once you reach adulthood, you decide who participates in your healthcare visits and who may receive your medical information.
Next Step Young Adult Health accepts the same insurance plans as other COPC primary care practices.
You’re always welcome to establish care with another adult primary care provider if you choose. The goal of Next Step Young Adult Health is to serve as your primary care home until you reach age 26, at which point we’ll help you establish care with a long-term adult medicine provider.
No. Because Next Step is part of COPC, your new care team will already have access to your existing COPC medical record.
Yes. If your COPC pediatric care ended within the past year and you haven’t established with a new primary care provider, you can schedule at Next Step.
