Homebound Vaccination Program Helps Vulnerable Patients

As a trailblazer for leading healthcare innovations, we are pioneering care that enables our patients to have support from the entire COPC team - inside and outside of the primary care office.

When faced with COVID-19 vaccine-related hurdles, COPC has gone above and beyond to hold clinics to ensure our community had timely access to vaccines. Our efforts did not stop there. We recognized that our vulnerable, homebound patients were unable to be vaccinated due to logistical challenges, so we piloted a program in March to vaccinate these patients in their homes. When faced with COVID-19 vaccine-related hurdles, COPC has gone above and beyond to ensure our community had timely access to vaccines. Many aging seniors COPC cares for would like to receive a COVID vaccine, but are unable to get to a clinic because they are physically home-bound or too sick to leave their home. Our team piloted the home delivery effort with ten CHPC patients and then rallied a group of team members to deliver vaccines to home-bound patients.
