worldwide. Early detection and diagnosis can be invaluable towards decreasing
patient and caregiver stress, better anticipating and preparing for the consequences
of the disease, while beneficially impacting quality of life.

Our specialty consult program is dedicated to providing support to patients with dementia
and cognitive decline. We will collaborate with your loved ones and COPC primary care
physician to provide highly coordinated care.
Our goal is to give our patients and their loved ones a resource for timely diagnosis, second opinions, medication review, functional assessments, treatment plans, and identification of community resources to help manage the condition.
What to Expect from the Initial Visit
A cognitive and/or neuropsychological evaluation
A functional assessment
A medication, lab work, and radiology/imaging review
A physical examination
A summary care conference to discuss initial impressions, treatment recommendations, and next steps
Each initial visit will be approximately 90 minutes. We ask that patients be accompanied by a family member or close friend.
